Functional neurological disorder

FND symptom diary

Record functional seizures, tremor, weakness and other FND symptoms as they happen, then give your neurologist a clear picture of what's been going on between appointments.

Coming soon to iPhone and Android.

What to record for FND

The FND template asks about each episode, so the details are written down while they're fresh. Fill in what you can and skip anything you don't know. You can always come back later.

  • What happened, in your own words
  • Where in your body, on a body map
  • How it started: suddenly, or with warning signs
  • What you were doing, and where you were
  • The time it started
  • How long it lasted, or that it's still going
  • How long any shaking went on
  • What stopped it, when and how
  • How you felt afterwards
  • What a witness saw

For when you can't stop to write

Start the timer, fill in the rest later

One tap starts the clock. Tap again when it stops, and Kind Witness saves exactly how long it lasted.

Let a witness fill it in

During a functional seizure you may lose awareness, or not remember what happened. The witness card asks the person with you what they saw.

Voice notes, then video

Coming in later updates, for the times when typing is too hard. Until then, note in the entry if someone filmed it.

Note what was going on

Tap any possible triggers on an entry. Over time, your report shows how often each one came up, as plain counts.

Questions about FND diaries

Does Kind Witness diagnose FND?

No. Kind Witness is a diary. It records what you tell it and helps you share it. FND is diagnosed by a doctor, from your history, your symptoms and an examination, sometimes with tests to rule out other conditions.

Can I track FND alongside epilepsy, POTS or migraine?

Yes. Choose every condition that applies when you set up. Each entry uses the template you pick, and reports can show each condition on its own or all together.

Should someone film my episodes?

A good home video can help a neurologist, alongside what you tell them. If it's safe, the person with you can film once you've had first aid. Ask your neurologist how they'd like to receive it. How to record a seizure for your neurologist

Is my FND diary private?

Your diary is stored and encrypted on your phone. Nothing leaves it unless you send it. How we protect your diary

Is there a paper version?

Yes. The free printable symptom diary asks the same basic questions as the app, so you can fill it in by hand. Printable diaries
