Patient diaries you can read in two minutes

Kind Witness helps patients with episodic and long-term conditions arrive with dated records instead of memories. Here's what you'll receive, and what it can and can't tell you.

What's in a report

  1. One summary page

Date range, conditions, allergies, counts for each condition, the longest episode, a time of day breakdown, quiet days, and the patient's questions for you.

  2. A dated timeline

Every entry in order, with its start time and length, and whether it was still going when recorded.

  3. Each entry in full

Symptoms, body location, how it started, what the patient was doing, what stopped it, possible triggers and severity on their own scale.

  4. Witness accounts, kept apart

What someone else saw is marked as theirs, with their name and relationship, never mixed into the patient's own words.

  5. Photos with dates

Thumbnails beside each entry, and a full-size appendix when the patient includes one. Photos are never filtered or tinted.

  6. Their medicine list

The patient's current medicine list, as they entered it, and any doses they marked as missed.

What Kind Witness is not

We'd rather be clear about the limits than have a report taken for more than it is.

Not a clinical record

Everything is recorded by the patient or a witness, and every page says so.

Not monitoring

Kind Witness sends no alerts, and nobody watches the data between appointments.

Not diagnostic

It counts and lists. It never interprets, predicts or scores.

Not a medical device

It's a record-keeping tool, and is not intended to diagnose, treat or prevent any condition.

Suggesting it to patients

  • Free for patients, with no account, on iPhone and Android
  • Printable diaries for patients who prefer paper
  • Reports arrive as a PDF from the patient's own email. Ask them for a shorter date range, or to leave photos out, if that suits you better.
  • Bulk Pro codes for clinics and charities
